Job Summary

The Behavioral Health Crisis Case Manager plays a vital role in the Emergency Department as part of a multi-disciplinary team to provide crisis evaluation, service linkage, and meet post-discharge needs of patients. This position requires collaboration with the medical and psychiatric teams to provide optimal patient care.

Key Responsibilities
  • Coordinate care management with agencies such as Adult Protective Services, DCFS, SASS, psychiatric units for admissions, and insurance companies as needed.
  • Provide crisis evaluation and service linkage to patients and their families.
  • Meet post-discharge needs of patients, ensuring a smooth transition to community-based services.
  • Collaborate with the medical and psychiatric teams to provide optimal patient care.
Requirements
  • Master's Degree or higher in a behavioral health field.
  • A high level of communication skills is required to interact effectively with others in very difficult situations.
  • Knowledge of DSM5 diagnostic criteria, ICD10, and interqual/MCG standards.
  • A minimum of two years of previous assessment experience.
Preferred Qualifications
  • Minimum of two years of experience in crisis/emergency services, behavioral health setting.
  • Knowledge of patient rights and law pertaining to mental health.
