Role Overview

The Patient Care Coordinator plays a crucial role in managing care among our case team members. This position involves:

  • Facilitating communication between patients, caregivers, physicians, and field staff to ensure personalized care.
  • Coordinating care across various disciplines to uphold quality standards.
  • Timely updates to all relevant team members regarding documentation, orders, and patient status changes.
  • Monitoring communications and rehabilitation plans from Clinical Managers to meet individual patient needs.
  • Implementing care plans and adjusting them as necessary in collaboration with Clinical Managers and physicians.
  • Recognizing changes in patient conditions and initiating appropriate actions in consultation with healthcare professionals.
  • Leading efforts to integrate and coordinate care with facility staff, pharmacies, and supervisors.
  • Adhering to company policies and regulatory standards in all aspects of care delivery.
  • Participating in staff meetings and ongoing training sessions.
  • Completing all documentation accurately and in compliance with Medicare guidelines.
  • Engaging in on-call and weekend rotations as required.

Qualifications

  • Experience in home health nursing is preferred.
  • Ability to communicate effectively, both verbally and in writing.
  • Willingness to travel as needed.
  • Physical capability to perform light to moderate activities, including lifting and assisting clients.
