Position Overview

This role involves delivering high-quality, empathetic, and supportive patient care. The Patient Care Coordinator will assist with administrative tasks related to patient check-in and check-out processes, ensuring outstanding customer service through both in-person and telephone interactions.

Key Responsibilities:

  • Under the guidance of the Nurse Manager, the Patient Care Coordinator will provide skilled, direct, and personalized care, which includes, but is not limited to, measuring patient vital signs and preparing examination rooms.
  • Maintain a clean, organized, and safe work environment.
  • Perform front office duties to ensure patients are attended to promptly and effectively.
  • Support the team by managing patient check-ins, directing visitors, making reminder calls, and handling incoming calls.
  • Facilitate patient check-outs and provide summaries of care to patients.

Qualifications:

  • Ability to interact with patients and colleagues in a professional and courteous manner.
  • Capable of working independently as well as collaboratively within a professional team.
  • Strong attention to detail and commitment to maintaining the confidentiality of sensitive information.
  • Excellent organizational skills and the ability to manage multiple tasks efficiently.
  • Understanding and adherence to Medical Record Policies and Procedures.
  • Preferred experience of six (6) months in a healthcare setting, though not mandatory.
  • General office skills with prior office experience.
  • High school diploma or equivalent.
