We are seeking a dedicated Registered Nurse (RN) to join our Case Management team at Mass General Brigham in Cambridge, Massachusetts.

Responsibilities
  • Evaluate patient needs and develop effective care plans
  • Collaborate with healthcare teams to ensure coordinated care delivery
  • Analyze data and identify opportunities for process improvement
  • Develop and maintain relationships with patients, families, and healthcare providers
Requirements
  • Demonstrated experience as a RN Case Manager or similar role
  • Licensed RN in the state of Massachusetts
  • Minimum 2 years Med/Surg experience
  • Case Manager certificate preferred
  • ADN required; BSN strongly preferred
