About the Role:

  • We are seeking an experienced Outpatient Case Manager to join our team, with a focus on delivering high-quality care and improving patient outcomes.
  • Responsibilities include assessing member needs, developing care plans, and coordinating services to ensure optimal health outcomes.

About Our Benefits:

  • We offer a competitive salary range of $45-$50 per hour for Registered Nurses and $37.50-$40 per hour for Licensed Vocational Nurses.
  • In addition to base pay, we provide a comprehensive benefits package, including medical, dental, and vision insurance, 401(k) eligibility, and paid time off.

Requirements:

  • A graduate from an accredited Registered Nursing Program or Licensed Vocational Nursing Program.
  • Current CA RN or LVN license, current CPR certification, valid CA Driver's license.
  • At least 3 years of acute care or case management experience, preferably in an HMO setting.
  • Typing speed of 40 words per minute with accuracy.
