Job Summary

We are seeking a skilled and compassionate Outpatient Case Manager RN to join our team at Regal Medical Group, Inc.

Key Responsibilities:
  • Clinical Assessment: Perform thorough assessments of members to determine their acuity score and identify necessary scheduled follow-up.
  • Plan Development: Develop outcome-based plans of care based on member input and assessed needs, implementing and evaluating the plan as needed.
  • Documentation: Accurately document member assessments, reassessments, care plans, and other pertinent information in the member's medical record.
  • Community Visits: Conduct home visits or hospital visits as needed to assess patient progress and meet with care team members.
  • Transition Management: Identify planned and unplanned transitions of care, educate members and caregivers on transition processes, and manage care transitions from sending to receiving settings.
  • Collaboration: Effectively communicate with physicians, case managers, and other healthcare professionals to coordinate care and ensure seamless transitions.
Requirements:
  • Education: Graduate from an accredited Registered Nursing Program or Licensed Vocational Nursing Program.
  • Licensure: Current CA RN or LVN license, current CPR certification, and valid CA Driver's License.
  • Experience: 3 years acute care or case management experience preferred, with typing skills and proficiency in MS Office programs.

Salary Range

The estimated salary for this position is between $45.00 - $50.00 per hour depending on location, experience, and licensure. The total compensation package may also include additional elements such as a sign-on bonus and discretionary awards, as well as full range of medical, financial, and/or other benefits.
