Job Summary

We are seeking a skilled Clinical Care Coordinator to join our team in an acute inpatient setting. As a key member of our healthcare team, you will play a vital role in coordinating patient care and ensuring seamless transitions throughout the hospital stay.

Key Responsibilities
  • Patient Assessment and Care Planning: Conduct thorough assessments of patients' medical needs and develop personalized care plans in collaboration with interdisciplinary teams.
  • Care Coordination: Coordinate with physicians, nurses, and other healthcare professionals to ensure timely and effective delivery of patient care services.
  • Discharge Planning: Develop and implement discharge plans that address patients' ongoing medical needs and facilitate smooth transitions to post-acute care settings.
  • Communication: Effectively communicate with patients, families, and caregivers to ensure they are informed and engaged in their care.
Requirements
  • Education and Experience: Graduate from an accredited Registered Nursing Program or Licensed Vocational Nursing Program; current California Licenses; knowledge of Healthcare and Managed Care preferred.
  • Skills and Qualifications: Typing 40 words per minute with accuracy; knowledge of computers, faxes, printers, and all other equipment; knowledgeable in MS Office programs (i.e., Word, Excel, Outlook, Access, and Power Point); ability to deal with responsibility with confidential matters.
Compensation and Benefits

The estimated salary for this position is $45-$50 per hour.
