Regal Medical Group, Inc. is a dynamic and rapidly growing Independent Physician Association in Southern California, seeking a highly skilled Inpatient Case Manager to join our team.

The ideal candidate will have a strong background in clinical experience, particularly in the acute care setting, with a proven track record of effective case management. As an Inpatient Case Manager, you will play a crucial role in assessing patients' needs, developing personalized plans of care, and ensuring seamless transitions throughout their hospital stay.

We offer a competitive salary range of $43,400 - $50,000 per year, based on experience and qualifications, as well as a comprehensive benefits package that includes employer-paid medical, dental, and vision insurance, flexible spending account, employee assistance program, and more.

Job Description:

  • Clinical Review and Assessment: Conduct thorough clinical reviews upon admission and daily while patients are hospitalized to ensure they receive the highest level of care.
  • Level of Care Determination: Determine the appropriate level of inpatient care for each patient, taking into account their specific needs and circumstances.
  • Monitoring and Intervention: Continuously monitor patients' progress and intervene as necessary to prevent delays in services or identify potential barriers to safe transition.
  • Discharge Planning: Develop and implement safe discharge plans, coordinating with internal departments and external healthcare providers to ensure a smooth transition to outpatient management.

Required Skills and Qualifications:

  • Education: Graduate from an accredited Registered Nursing Program or Licensed Vocational Nursing Program.
  • License: Current California License.
  • Experience: At least 2 years of clinical experience in the acute care setting and discharge planning experience.
  • Skillset: Strong typing skills (40 words per minute), knowledge of MS Office programs, and ability to handle confidential matters.
