About the Role:

The successful candidate will work closely with our team to assess, plan, implement, and evaluate the care of high-risk members. As a Medical Case Manager, you will be responsible for coordinating services, communicating with healthcare providers, and facilitating transitions of care to ensure seamless continuity of treatment.

Key Responsibilities:
  • Care Coordination: Assess patients' needs, develop individualized care plans, and implement strategies to achieve optimal health outcomes.
  • Communication: Collaborate effectively with healthcare teams, patients, and families to ensure timely and accurate communication.
  • Transition Management: Facilitate smooth transitions between healthcare settings, ensuring that patients receive the necessary care and support during critical periods.
  • Patient Education: Educate patients and caregivers on healthy habits, disease management, and medication adherence.
Requirements:
  • A current RN or LVN license in California.
  • 3 years of acute care or case management experience preferred.
  • Knowledge of MS Office programs and computer skills.
  • Familiarity with HMO experience and utilization review.
  • Typing speed of 40 words per minute with accuracy.
