Homeless Resource Outreach Specialist

We are seeking a highly motivated and compassionate Homeless Resource Outreach Specialist to join our team. As a key member of our outreach team, you will be responsible for building relationships with homeless individuals, connecting them with essential resources and services, and advocating for their needs.
Key Responsibilities- Build and maintain relationships with homeless individuals, service providers, and community stakeholders
- Connect homeless individuals with resources and services, including emergency shelters, substance abuse treatment, and mental health services
- Provide crisis intervention, harm reduction, and conflict resolution services as needed
- Collaborate with community organizations, government agencies, and other stakeholders to identify and address gaps in services
- Document and record client interactions, including triage of individual needs, contacts, referrals, and site conditions
- High school diploma or equivalent required; one year of experience working in outreach or homelessness services preferred
- Valid driver's license required; willingness to travel throughout Metro Louisville
- Ability to work independently and as part of a team; strong communication and interpersonal skills
- Basic computer literacy, including Microsoft Suite applications
- Proficiency with HMIS, ArcGIS, and Microsoft Suite applications
