Job Summary

The Care Manager will work directly with parents and their families, providing coaching and support to help them navigate the challenges of trauma and adversity. This role requires a strong understanding of trauma-informed care, family systems, and the ability to work collaboratively with interdisciplinary teams.

Responsibilities
  • Conduct trauma-informed parent coaching through in-person or telehealth sessions
  • Utilize a family-systems approach to support clients
  • Process client applications and maintain accurate records
  • Enter and maintain client contact and demographic data in our client management system
  • Complete clinical assessments as needed and create actionable plans for client families
  • Coordinate with interdisciplinary teams to meet the needs of client families
  • Provide crisis support and management as needed
  • Build and maintain relationships with case workers to promote Chosen's services and generate referrals
  • Participate in outreach events to educate potential referral sources about our mission and services
Requirements
  • Master's degree in social work or a related field
  • 2+ years of experience in social services and/or mental health
  • LCSW, LMFT, or LPC license strongly preferred
  • Professional experience working with foster or adoption children and families strongly preferred
  • Trauma-Informed training or certification
  • Solid working knowledge of current industry norms and challenges
  • Proactive, resourceful, and a problem-solver
  • Strong personal integrity
  • Demonstrated experience in client support and direct client communications
