Job SummaryThe Care Manager will work directly with parents and their families, providing coaching and support to help them navigate the challenges of trauma and adversity. This role requires a strong understanding of trauma-informed care, family systems, and the ability to work collaboratively with interdisciplinary teams.
Responsibilities- Conduct trauma-informed parent coaching through in-person or telehealth sessions
- Utilize a family-systems approach to support clients
- Process client applications and maintain accurate records
- Enter and maintain client contact and demographic data in our client management system
- Complete clinical assessments as needed and create actionable plans for client families
- Coordinate with interdisciplinary teams to meet the needs of client families
- Provide crisis support and management as needed
- Build and maintain relationships with case workers to promote Chosen's services and generate referrals
- Participate in outreach events to educate potential referral sources about our mission and services
- Master's degree in social work or a related field
- 2+ years of experience in social services and/or mental health
- LCSW, LMFT, or LPC license strongly preferred
- Professional experience working with foster or adoption children and families strongly preferred
- Trauma-Informed training or certification
- Solid working knowledge of current industry norms and challenges
- Proactive, resourceful, and a problem-solver
- Strong personal integrity
- Demonstrated experience in client support and direct client communications
- Competitive salary range: $50,000-$55,000 per year
