Position Overview:
This role is essential in managing and facilitating the services provided by the Medical-Financial Partnership Program within various healthcare settings, including prenatal, pediatric, and family medicine. The primary responsibilities include:

Key Responsibilities:
1. Oversee and coordinate the activities of the Medical-Financial Partnership team to ensure seamless service delivery.
2. Enhance continuity of care and minimize service gaps for families.
3. Implement a two-generation resilience-building intervention within clinical environments.

Clinical Support:
Provide direct clinical assistance to the interdisciplinary team, fostering trust-based relationships between parents and providers. This includes:
- Supporting parents in enhancing their parenting skills, particularly in high-risk situations.
- Collaborating with the team to promote secure attachments during prenatal and pediatric stages, addressing various health determinants.

Data Collection and Program Support:
Assist in gathering clinical measures to support patient care and program evaluation. Responsibilities also include:
- Delivering professional social work services to families with complex needs in an outpatient setting.
- Supervising and educating social work interns and resilience coaches, ensuring adherence to practice guidelines.

Collaboration and Communication:
Maintain effective communication with various healthcare professionals and community organizations to facilitate service connections. This involves:
- Documenting clinical and program activities accurately and promptly.
- Participating in multidisciplinary team meetings to assess client progress and develop care plans.

Qualifications:
- A Master's Degree in Social Work from an accredited institution, with a minimum of two years of clinical experience.
- A valid Licensed Clinical Social Worker license from the appropriate regulatory body.
- Strong experience in a multidisciplinary medical setting, particularly in assessing psychosocial needs.

Desirable Skills:
- At least five years of post-degree experience in early childhood mental health and intervention planning.
- Ability to thrive in a dynamic environment with minimal supervision.
- Proficiency in Spanish is a plus.
