  • Availability during standard Monday to Friday primary care hours

Key Responsibilities:

  • Perform comprehensive nursing evaluations, including social, medical, mental health, substance use, and medication histories
  • Deliver ongoing care management, chronic disease support, and patient education
  • Work in conjunction with physicians to manage substance use disorder medications and coordinate patient programs
  • Ensure precise and timely documentation within electronic health records
  • Engage in population management through proactive outreach and case reviews
  • Provide health education, counseling, and risk reduction strategies to patients involved in interventions
  • Maintain a professional environment within a multi-provider healthcare setting
  • Advocate for patients across all age groups based on nursing standards and judgment
  • Perform duties within the RN scope of practice and adhere to legal regulations
  • Involve patients in the development of their treatment plans
  • Implement trauma-informed care practices and de-escalation techniques
  • Comply with patient confidentiality regulations, including HIPAA
  • Address inquiries and direct questions to the appropriate personnel

Compensation: $112,500 annually
