Job Description

Aniz Inc is seeking a highly skilled and compassionate Case Manager to join our team. As a Case Manager, you will play a critical role in ensuring the well-being and success of our clients.

Key Responsibilities:
  • Client Assessment and Care Planning: Conduct thorough assessments of clients' needs and develop personalized care plans to achieve their goals.
  • Treatment Program Coordination: Collaborate with interdisciplinary teams to coordinate and implement treatment programs, ensuring seamless delivery of services.
  • Client Support and Advocacy: Provide emotional support and advocacy for clients, connecting them with necessary resources and services.
  • Case Management and Monitoring: Regularly review and update client records, monitoring progress and making adjustments to care plans as needed.
  • Communication and Collaboration: Maintain open communication with clients, families, and healthcare professionals, ensuring a cohesive and effective treatment approach.
Requirements:
  • Education: Master's degree in Social Work or related field preferred.
  • Experience: 1+ year of experience in a Rapid Rehousing program or similar setting.
  • Skills: Excellent documentation skills, strong analytical and decision-making abilities, and effective communication and interpersonal skills.
