Role Overview
As a Licensed Nurse Case Manager, you will play a crucial role in assisting program recipients, their families, and caregivers in navigating healthcare benefits. Your responsibilities will include:

  • Developing and implementing program policies and procedures.
  • Managing the complete patient care cycle, including assessment, planning, implementation, monitoring, and evaluation of health and human services.
  • Coordinating and delivering care that is safe, timely, effective, efficient, equitable, and centered on the client.
  • Handling case assignments, drafting service plans, reviewing case progress, and determining case closure.
  • Conducting periodic in-person assessments throughout the state.
  • Facilitating various aspects of care, including case coordination and information sharing.
  • Advocating for patients by helping them make informed decisions regarding their clinical status and treatment options.
  • Building effective working relationships with the medical team throughout the case management process.
  • Monitoring patient progress and ensuring satisfaction through regular interactions.
  • Documenting case information accurately and producing statistical reports.
  • Promoting quality and cost-effective interventions and outcomes.
  • Addressing motivational and psychosocial issues.
  • Adhering to professional standards as per established protocols, rules, and regulations.

Qualifications
The ideal candidate will possess:

  • Excellent communication, interpersonal, and client-facing skills.
  • The ability to work independently as well as collaboratively within a team.
  • Proven experience in case management, particularly in nursing, medical, or mental health care.
  • A strong understanding of case management principles, healthcare management, and reimbursement processes.
  • Experience with the psychological aspects of care.
  • Outstanding organizational and time management skills.
  • Familiarity with emerging professional and technical knowledge.
  • Effective problem-solving skills and the ability to manage multiple tasks.
  • A compassionate approach with strong teamwork capabilities.
  • A BS degree in a related healthcare field is preferred.
  • Experience working with pediatric populations and relevant training in health or social services.
  • A current license, certification, or registration as a Registered Nurse or Social Worker.

Compensation and Benefits
Compensation for this role ranges from $74,100 to $119,500, depending on various factors including location, role, skill set, and experience. In addition, PCG offers a comprehensive benefits package that includes medical and dental care, 401k, paid time off, parental leave, and bereavement leave.
