- Collaboration: Work closely with medical directors, physician advisors, and other healthcare professionals to develop and implement care management treatment plans.
- Problem-Solving: Assist in problem-solving with providers, claims, or service issues to ensure seamless care delivery.
- Communication: Effectively communicate with members, families, and healthcare providers to ensure informed decision-making and optimal care outcomes.
- Documentation: Maintain accurate and timely documentation of member interactions, care plans, and outcomes to ensure compliance and quality standards.
- Education: Bachelor's degree in a health-related field or equivalent experience.
- Experience: Minimum of 3 years of clinical experience, with a focus on case management or a related field.
- Licensure: Current, unrestricted RN license in applicable state(s) required.
- Certification: Certification as a Case Manager is preferred.
- Oncology or Home Care Experience: Previous experience in oncology or home care settings is preferred.
- Strong Communication Skills: Excellent verbal and written communication skills, with the ability to effectively communicate with diverse stakeholders.
- Analytical Skills: Strong analytical skills, with the ability to assess complex patient needs and develop effective care plans.
