Job Summary

We are seeking a skilled Clinical Care Coordinator to join our team at Porter Hospital. As a key member of our clinical staff, you will play a vital role in ensuring the highest level of patient care and satisfaction.

Key Responsibilities
  • Assist providers in the care of patients, coordinating all care activities to maximize provider time in the clinical area.
  • Provide direct patient care, evaluating interventions and consulting with providers as necessary to ensure optimal patient outcomes.
  • Perform triage of patients per protocols established by the physician and/or organization.
  • Prepare patients for provider examinations and assist with procedures and other processes.
  • Provide patient education and coordinate educational activities.
  • Collect patient health information and communicate it to appropriate health professionals.
  • Arrange consults, tests, and procedures, and verify orders for patients.
  • Demonstrate safe medication and treatment administration.
  • Identify patients and take responsibility for individual actions.
  • Adhere to laboratory quality control policies and document quality control activities.
  • Oversee the training and performance of clinical staff.
  • Deliver care in a nonjudgmental, nondiscriminatory, and ethical manner.
Requirements
  • Associate or Bachelor's Degree in Nursing from an accredited college or university.
  • Registered Nurse License in the State of Indiana.
  • Current CPR Certification preferred, required within 3 months of hire.
  • Minimum of one-year experience in a physician office RN role preferred.
