Northwest Health - Porter is seeking a Nurse Practitioner to work in a diverse role in both select community long term care facilities as well as the primary care office.

The goal is to ensure timely assessment and intervention as needed.

Ideal candidate has strong clinical experience and desire to work with patients of all ages in health maintenance and disease prevention.


Details of the position include:
Combination of both nursing home and primary care practice
Round in nursing homes for well visits and in response to patient needs
Opportunity to develop primary care practice for new and existing patients
Strong communication and follow through needed with nursing staff, physicians and
families
