As a Care Navigator (Medical Assistant), you will serve as a vital link between our patients and the healthcare system, often being the first point of contact for new members. This role exemplifies our dedication to accessible healthcare.
Your responsibilities will include managing essential components of each patient's personalized care plan, which encompasses both clinical and administrative support. You will triage patients, schedule appointments, and prepare them for examinations.
Through meaningful one-on-one interactions, you will have a direct impact on the lives of individuals, helping them acclimate to a new model of primary care.
The Care Navigator is a crucial member of a team committed to delivering outstanding and comprehensive primary care to a specific patient demographic.
Key Responsibilities:
- Schedule telehealth appointments, triage patients remotely, and document patient interviews, histories, and chief complaints using Ascend's electronic medical records (EMR) system. Manage prescription refills.
- Facilitate telehealth consultations by reviewing charts in advance, ensuring all necessary equipment is functional, guiding patients through the technology, and assisting in assessments during telehealth encounters.
- Prepare patients for examinations and coordinate referrals for routine screenings; assist physicians during exams and clarify treatment procedures and instructions to patients.
- Collaborate closely with the Care Coordinator to ensure all patients have established care plans and follow-up appointments as directed by the provider.
- Manage incoming and outgoing communications, including calls, chats, and emails as assigned.
- Provide guidance and support to both new and existing members, educating them on the benefits of Ascend Medical and assisting them in navigating their health insurance to maximize available benefits.
- Process patient messages, online inquiries, follow-up calls, and route them to appropriate team members; engage in secure messaging with patients to gather and relay important information to the physician.
Key Performance Outcomes:
- 100% of consent forms collected from patients
- 100% of credit card payments processed prior to appointments
- 80% reappointment rate for new patients
- 60% reappointment rate for existing patients
- 100% follow-up on labs, referrals, and prescriptions
- 85% answer rate for patient communications
- 80% show rate for appointments

Requirements:
A high school diploma or equivalent is required. Graduation from a nationally accredited Medical Assistant program or equivalent healthcare training is necessary. Certification as a Registered/Clinical Medical Assistant (CCMA or CRMA) is preferred. A minimum of 2 years of experience as a Medical Assistant in a healthcare setting is required. Current CPR certification is mandatory.
Knowledge, Skills, and Abilities:
- Understanding of clinical procedures, medical terminology, anatomy, physiology, ICD-10 and CPT coding, insurance eligibility, and copay determination.
- Exceptional communication skills, utilizing appropriate medical terminology and adhering to legal and ethical standards.
