Job Title: Patient Care Coordinator

Join the Georgia Department of Public Health team as a Patient Care Coordinator and make a meaningful impact in the lives of Georgians.

About the Role

This is a full-time salaried position that requires a Bachelor's degree in a related field and two years of experience in social service delivery. The ideal candidate will have excellent customer-focused interpersonal skills, excellent verbal and written communication skills, and the ability to maintain HIPAA compliance and client confidentiality.

Responsibilities
  • Acts as client advocate to coordinate required services or resolve crisis situations
  • Counsels clients in individual and group sessions to help them overcome dependencies, recover from illness, and adjust to life
  • Identifies environmental impediments to client progress through interviews and review of patient records
  • Monitors, evaluates, and records client progress according to measurable goals described in treatment and care plans
  • Organizes support groups or counsels family members to assist them in understanding, dealing with, and supporting the client
  • Performs clinical assessments of the behavioral health needs of consumers presenting for services
  • Provides case management services related to identified treatment needs
  • Provides training to direct care staff in implementing behavior modification techniques and treatment plans
  • Refers clients or family members to community resources to assist in recovery from mental or physical illness and to provide access to services such as financial assistance, legal aid, housing, job placement, or education
  • Receives additional training, as required, to gain full proficiency and experience in all areas

Requirements
  • Bachelor's degree in a related field from an accredited college or university
  • Two years of experience in social service delivery
  • Excellent customer-focused interpersonal skills
  • Excellent verbal and written communication skills
  • Ability to maintain HIPAA compliance and client confidentiality
Preferred Qualifications
  • Excellent computer skills, experience with electronic medical records a plus
  • Experience with CAREWare/HMIS a plus
  • Bilingual in Spanish/English, required
