Hospice Care Coordinator

We are seeking a skilled Hospice Nurse Navigator to join our team at Mount Carmel Health System. As a Hospice Nurse Navigator, you will play a critical role in providing high-quality patient care and support to individuals and their families in their homes.
Key Responsibilities:
- Assess and evaluate patient care needs, developing and implementing individualized plans of care that address physical, emotional, social, and spiritual needs.
- Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and communication.
- Provide education and support to patients and families on hospice care, pain management, and end-of-life care.
- Conduct regular home visits to assess patient needs, provide care, and address any concerns or issues.
- Document patient information, progress, and outcomes in accordance with regulatory requirements and organizational policies.
- Participate in interdisciplinary team meetings to discuss patient care plans, share knowledge, and address any challenges or concerns.
- Stay up-to-date with industry developments, best practices, and regulatory requirements to ensure high-quality patient care.
Requirements:
- Minimum of 2 years of experience as a professional nurse, preferably in a community home health or hospice setting.
- Current licensure to practice as a registered nurse in the state of Ohio.
- Ability to operate a motor vehicle and possess a valid driver's license from the State of Ohio.
- Strong communication, interpersonal, and problem-solving skills.
- Ability to work independently and as part of a team.
