Hospice Nurse Specialist

At Georgia Hospice Care, we're seeking a dedicated and compassionate Registered Nurse to join our team. As a Hospice Registered Nurse, you'll play a vital role in providing high-quality patient care and support to patients and their families.
**Key Responsibilities:**
* Provide skilled nursing care to patients in their homes, focusing on comfort, dignity, and quality of life
* Collaborate with the interdisciplinary team to develop and implement individualized care plans
* Conduct thorough assessments, identify patient needs, and coordinate care with other healthcare professionals
* Educate patients and families on disease management, symptom control, and end-of-life care
* Document patient progress, identify areas for improvement, and contribute to quality improvement initiatives
**Requirements:**
* Graduate of an accredited school of nursing with a current state license as a registered nurse
* 2+ years of nursing experience in a clinical care setting (hospice experience preferred)
* Reliable transportation
* Ability to sit, stand, bend, move intermittently, and lift at least 25 lbs
