Responsibilities
  • Provide Direct Patient Care: Our team members provide direct patient care as defined in the State Nurse Practice Act.
  • Implement Current Nursing Practice: Our team members implement current nursing practice following a comprehensive assessment and the plan of care.
  • Provide Accurate Documentation: Our team members provide accurate and timely documentation of patient services to reflect the plan of care.
  • Assess and Educate Patients: Our team members assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care.
  • Coordinate Hospice Services: Our team members participate in the coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group.
  • Manage Pain and Symptoms: Our team members provide appropriate pain/symptom management, evaluating the patient's response to treatments/medications.
Qualifications
  • Graduate of an Accredited Program: Our team members must be graduates of an accredited practical nurse or vocational nursing program.
  • Three Years of Nursing Experience: Our team members must have three years of nursing experience, with community health, hospice, or medical/surgical experience preferred.
  • Current LPN/LVN License: Our team members must be currently licensed as an LPN/LVN in the practicing state.
  • CPR Certification: Our team members must possess current CPR certification.
  • Good Communication and Organization Skills: Our team members must demonstrate good verbal and written communication, and organization skills.
  • Valid Driver's License: Our team members must have a valid driver's license with an automobile that is insured in accordance with state requirements.
  • Understanding of Hospice Concept: Our team members must understand the philosophy of hospice concept and the needs of the terminally ill.
