Position Overview:

The Clinical Care Coordinator at The University of Kansas Hospital is a vital role that involves the application of nursing principles to facilitate a collaborative approach to patient care. This position is essential in enhancing and maintaining the health of patients through effective communication and teamwork with healthcare professionals, while also providing emotional and physical support to patients and their families.

Key Responsibilities:

  • Welcomes patients and prepares them for their medical evaluations or procedures.
  • Records comprehensive health histories and identifies any abnormal conditions for the medical team.
  • Measures vital signs and conducts necessary screenings as per established protocols.
  • Administers medications via intramuscular, subcutaneous, and oral routes in accordance with nursing standards.
  • Monitors, documents, and communicates patients' conditions and responses to treatments to the healthcare team.
  • Assists in the setup of medical supplies and equipment, supporting physicians during examinations and treatments.
  • Ensures accurate and timely documentation in patient charts, and manages the forwarding of these records as necessary.
  • Facilitates the scheduling of diagnostic tests and treatments, and coordinates referrals to specialists, including obtaining necessary pre-authorizations.
  • Collects and processes laboratory specimens, retrieves lab results, and enters relevant data into the electronic medical record (EMR) system.
  • Informs patients about test outcomes and follow-up appointments, referring them to registered nurses or physicians for educational needs.
  • Reviews medications, treatment plans, and after-visit instructions with patients and their families.
  • Handles incoming calls, directing inquiries to the appropriate healthcare provider.
  • Conducts pre-visit preparations by reviewing EMR for patient history and proactively ordering necessary tests.
  • Completes required documentation such as FMLA, DME, and Home Health orders, ensuring appropriate physician signatures are obtained.

Qualifications:

  • High School Diploma or GED required.
  • BLS Certification is mandatory.
  • Active Licensed Practical Nurse (LPN) license in the relevant state.
  • Basic typing and word processing skills are essential.
  • Strong analytical, critical thinking, and problem-solving abilities.
  • Current multistate LPN license in good standing or proof of application for such a license.

Preferred Qualifications:

  • Prior experience in a medical office setting.
