The Long-Term Services and Supports (LTSS) Care Coordinator RN plays a vital role in developing a personalized, goal-oriented, and culturally sensitive approach to ensure individuals receiving LTSS access services in a supportive, effective, efficient, timely, and cost-effective manner. This role encompasses care coordination, case management, disease management, discharge planning, transition planning, and addressing social determinants of health while facilitating integration into the community.

Key Responsibilities:

  • Evaluate, strategize, and execute care plans tailored to the individual, focusing on the most suitable and least restrictive care options.
  • Identify and initiate referrals to social service programs, encompassing financial, psychosocial, community, and state support services.
  • Oversee the person-centered service/support plan throughout the entire care continuum.
  • Ensure effective communication with all stakeholders, providing essential health-related information to guarantee timely and quality coordinated care and services.
  • Advocate for individuals and families to ensure their needs and preferences are fully represented and supported by the health care team.
  • Conduct home visits in collaboration with the individual and care team, which may include a community service coordinator.
  • Perform in-person visits to various settings, including nursing homes, assisted living facilities, hospitals, or homes.
  • Act as a resource for community care coordinators, as applicable.
In this role, you will be recognized and rewarded for your performance in a challenging environment that provides clear guidance on achieving success and offers opportunities for professional development in other areas of interest.

Required Qualifications:
  • Current, unrestricted independent licensure as a Registered Nurse in Kansas.
  • Minimum of 2 years of experience in a community health setting within a health care role.
  • At least 1 year of experience working with individuals with long-term care needs.
  • Minimum of 1 year of experience with individuals receiving services under one of the home and community-based waivers in Kansas.
  • Proficiency in MS Word, Excel, and Outlook for at least 1 year.
  • Willingness to travel within the assigned region to visit Medicaid members in their homes or other locations, including community centers, hospitals, or provider offices.
Preferred Qualifications:
  • Licensed Social Worker or relevant clinical degree.
  • Experience managing populations with complex medical or behavioral health needs.
  • Familiarity with electronic charting systems.
  • Experience in coordinating community resources.
  • Knowledge of TA Medicaid Waivers.
