The Long-Term Services and Supports (LTSS) Care Coordinator RN plays a crucial role in developing personalized, goal-oriented care plans that are culturally sensitive and logically structured. This position ensures that individuals receiving LTSS access services in a supportive, effective, efficient, timely, and cost-effective manner. Care coordination encompasses case management, disease management, discharge planning, transition planning, and addressing social determinants of health while facilitating community integration.
Key Responsibilities:
- Evaluate, strategize, and execute individualized care plans tailored to the unique needs of each individual, focusing on the least restrictive level of care.
- Identify and initiate referrals to social service programs, including financial, psychosocial, and community support services.
- Oversee the person-centered service/support plan throughout the entire care continuum.
- Communicate essential health-related information to all stakeholders to ensure timely and quality coordinated care and services.
- Advocate for individuals and families to ensure their needs and preferences are fully represented and supported by the health care team.
- Conduct home visits in collaboration with the individual and care team, which may include a community service coordinator.
- Perform in-person visits, which may involve nursing homes, assisted living facilities, hospitals, or private residences.
- Act as a resource for community care coordinators, if applicable.

Required Qualifications:
- Current, unrestricted independent licensure as a Registered Nurse in Kansas.
- A minimum of 2 years of experience in a community health setting within a health care role.
- At least 1 year of experience working with individuals with long-term care needs.
- 1+ years of experience with individuals receiving services through home and community-based waivers in Kansas.
- Proficiency in MS Word, Excel, and Outlook.
- Willingness to travel within the assigned region to visit Medicaid members in their homes or other settings, including community centers and hospitals.
- Licensed Social Worker or relevant clinical degree.
- Experience managing populations with complex medical or behavioral health needs.
- Familiarity with electronic charting systems.
- Experience in arranging community resources.
- Knowledge of TA Medicaid Waivers.
