Social Services Coordinator

At the Good Samaritan Society, we are seeking a dedicated and compassionate Social Services Coordinator to join our team. As a key member of our interdisciplinary team, you will play a vital role in providing assistance with activities, social, emotional, and economic concerns of our residents and their families.
Key Responsibilities:
- Assist residents with activities of daily living, including bathing, dressing, and grooming.
- Provide emotional support and counseling to residents and their families.
- Develop and implement care plans to meet the unique needs of each resident.
- Collaborate with the healthcare team to ensure seamless care coordination.
- Investigate and resolve resident grievances in a timely and professional manner.
Requirements:
- High school diploma or equivalent required.
- Prior experience in long-term care, social services, or human services preferred.
- Certified Nursing Assistant (CNA) certification required within six months of hire.
