Job Summary

The Clinical Care Coordinator at Capital Caring Health plays a pivotal role in providing and coordinating hospice care services to patients within their homes. Working collaboratively with an interdisciplinary team, this individual conducts initial nursing assessments and makes treatment decisions based on the patient's needs and wishes, as well as those of their family.

This role involves gathering essential contact information, reviewing and documenting the patient's medication list, and taking a thorough history of their medical condition.

We are seeking a skilled Clinical Care Coordinator to join our team at Capital Caring Health. This individual will be responsible for assessing patient needs, developing care plans, and collaborating with healthcare professionals to ensure high-quality patient outcomes.

A competitive salary of $65,000 - $85,000 per year is available for this position, commensurate with experience.

Required Skills and Qualifications
  • Bachelor's degree in Nursing or related field
  • Minimum 1-2 years of experience in hospice care or a related field
  • Excellent communication and interpersonal skills
  • Ability to work collaboratively with interdisciplinary teams
