Job SummaryThe Clinical Care Coordinator will be responsible for coordinating patient care within the scope of licensure, in accordance with established nursing standards and hospital policies and procedures. This includes performing specific nursing services designed to promote health, prevent illness and injury, and provide care based on standards established or recognized by the DC Board of Nursing.
Responsibilities:- Coordinate patient care activities with interdisciplinary teams to ensure continuity and efficiency of care
- Conduct assessments, develop and implement individualized care plans, and evaluate patient progress
- Maintain accurate and timely documentation of patient information, including medical records and incident reports
To be successful in this role, you will need to have a current LPN license from the District of Columbia, as well as BLS certification. A minimum of one year's prior experience in an acute care setting is preferred. ACLS certification is also desirable.
