Salary
The estimated salary for this role is between $34.00 and $50.92 per hour, commensurate with equity and experience. This competitive compensation package reflects our commitment to recognizing and investing in the value of our employees.

About the Role
We are seeking an experienced Clinical Care Coordinator to join our team at Johns Hopkins Medicine. As a key member of our home health department, you will specialize in coordinating home care services and resources for patients transitioning from hospital to home. This vital role acts as a discharge planning resource without providing hands-on clinical care. Your expertise will empower our patients to navigate their recovery journey with confidence and dignity.

Key Responsibilities
- Coordinate home care services and resources for patients transitioning from hospital to home
- Act as a discharge planning resource without providing hands-on clinical care
- Collaborate with interdisciplinary teams to ensure seamless transitions of care

Requirements
- Graduate of accredited nursing or allied health care professional program required (BS preferred)
- Licensure in clinical discipline (RN, PT, OT) in the State of Maryland
- Active CPR certification
- Two years of home care or home care discharge planning experience preferred
- Thorough knowledge of home care community resources, agency policy and procedures, federal and state regulations, and third-party regulations for reimbursement of home care services
