About the Role

We are seeking a compassionate and skilled Social Worker to join our team at The Pines at Bristol Center for Health & Rehabilitation. As a Social Worker, you will play a vital role in enhancing the quality of life of our residents by advocating for their well-being and ensuring their holistic needs are met.

Key Responsibilities
  • Conduct comprehensive psychosocial assessments to identify residents' social, emotional, and environmental needs, as well as strengths and preferences.
  • Collaborate with residents, their families, and the interdisciplinary team to develop individualized care plans that address social, emotional, and psychological concerns.
  • Provide counseling and emotional support to residents and their families, offering guidance on coping strategies, adjustment to care transitions, and decision-making processes.
  • Serve as a strong advocate for residents' rights and preferences, ensuring their voices are heard and respected in care planning and decision-making processes.
  • Connect residents and their families with community resources, support services, and advocacy organizations to address social, financial, and legal needs.
  • Provide crisis intervention and support during challenging situations, such as grief and loss, family conflicts, or changes in health status.

Requirements
  • Masters degree in social work from an accredited program.
  • Current licensure or certification as a Social Worker in the state of employment.
  • Previous experience in Long-Term Care, geriatrics, or a related field.
  • Strong assessment, counseling, and crisis intervention skills.
  • Excellent communication and interpersonal abilities.
  • Ability to work collaboratively as part of a multidisciplinary team.
  • Empathy, compassion, and a commitment to person-centered care.
