Job Title: Children's Care Coordinator

The New Horizon Counseling Center is seeking a highly skilled and compassionate Children's Care Coordinator to join our team. As a key member of our interdisciplinary team, you will provide critical support to families and children with serious emotional disturbance, chronic conditions, or trauma experiences.

Key Responsibilities:
  • Conduct ongoing assessments and evaluations to ensure the provision of necessary services for each case.
  • Coordinate and monitor all services, including medical, mental health, and educational wellness/success, through collaborative relationships.
  • Provide home and field visits to offer counseling, crisis intervention, referral, and advocacy services, as well as assistance with basic needs.
  • Work collaboratively with each child and family to develop written care plans and crisis plans, as needed.
  • Prepare all charting, assessment forms, accountability forms, and direct service requirements in accordance with state and city regulations.
  • Attend regular supervisory and case conference sessions, trainings, and other meetings, as assigned.
Requirements:
  • Bachelor's Degree Required; Master's Degree Preferred in a field such as Social Work, Psychology, Education, Rehabilitation, Human Services, Occupational Therapy, Counseling, Community Mental Health, Sociology, Physical or Recreational Therapy.
  • Training and successful certification as a CANS-NY Assessor needed within 30 days of start date.
  • Two years of related human services experience required for B.A. level candidates; one year of experience required for M.A. level candidates.
  • Excellent interpersonal, communication, and time management skills, along with English-language writing skills necessary to fulfill state and city regulations for record keeping.
  • Ability and willingness to regularly travel, in some instances with clients in agency vehicle to many locations using various modes of reliable and safe transportation.
Preferred Qualifications:
  • Knowledge of Community Resources, Medicaid, and other entitlements.
  • Knowledge of Trauma, Child welfare, and/or Child development.
  • Bilingual a plus.

Pay Range: Salaries start at $47,000 annually
