Job SummaryWe are seeking an experienced Advanced Provider (Nurse Practitioner or Physician Assistant) to join our team in Queens, NY (Astoria) region.
You will work under the supervision of a physician, providing primary care to homebound and palliative care patients.
Key Responsibilities- Provide high-quality care to primary and palliative patients in and around Queens, NY
- Assess patient health through physical examinations, medical histories, and creating personalized care plans
- Effectively communicate with office staff and clinical operations across various internal platforms
- Develop impressions, diagnoses, and treatment plans
- Foster trust and consult with patients, families, and caregivers during visits
- Order diagnostic testing as medically necessary
- Prescribe medications, supplies, and interventions as needed
We are looking for a Board Certified Advanced Provider with a valid NP or PA certification in the State of New York.
Strong clinical skills, a positive demeanor, and excellent people skills are essential.
1-2 years of primary care experience with geriatric patients is preferred, but not required.
