- Develop and implement patient management care plans, updating them as needed to reflect changes in condition or new information.
- Collaborate with patients, family members, and healthcare teams to ensure effective communication and coordination of care.
- Prescribe diagnostics and interventions to prevent unnecessary hospital admissions and promote optimal patient outcomes.
- Participate in ongoing quality improvement initiatives to enhance patient care and clinical excellence.
- Completion of a Nurse Practitioner accredited program.
- Current state RN/NP license and certification through the American Academy of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC).
- Access to reliable transportation and ability to comply with Optum Motor Vehicle Safety policy.
- 2+ years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care.
- Knowledge of long-term care, community resources, and cost-effective alternatives for patient care delivery systems.
- Knowledge of CPT, ICD-10, and HCPC codes.
- Awareness of NCQA requirements, CMS guidelines, Medicaid/Medicare contracts, and benefit systems.
