Ambulatory Clinical Pharmacist

We are seeking a highly skilled and dedicated Ambulatory Clinical Pharmacist to join our team at Ochsner Health. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and contributing to the success of our organization.
Responsibilities- Provide pharmacokinetic consultation and evaluation of medication therapy for assigned patients.
- Monitor and evaluate patient response to drug therapy, documenting interventions and conducting outcome studies/evaluations.
- Develop and implement pathways, best practices, and guidelines for assigned populations.
- Educate staff, students, patients, and healthcare providers on medication management and other relevant topics.
- Mentor residents, students, and new staff to meet departmental goals.
- Collaborate with healthcare teams to ensure seamless transitions of care and optimal patient outcomes.
- Current Pharmacy license in the state of practice.
- Completion of a PGY1 Pharmacy Practice Residency (or 3 years of related specialty experience).
- Preferred: Doctor of Pharmacy (PharmD) and/or completion of a PGY2 Specialty Pharmacy Residency.
