Mental Health Nurse Manager

VNA Health Group is seeking a skilled Registered Nurse to fill the role of Home Health Manager, Mental Health. This position involves providing comprehensive care to patients experiencing mental illness, both acute and chronic disease states.
Key Responsibilities:
- Identify patient needs and develop a holistic plan of care.
- Evaluate patient progress and adjust the plan as needed.
- Collaborate with community agencies to coordinate patient care and facilitate discharge planning.
- Communicate with the Manager to ensure patient needs are met.
Qualifications:
- Completion of an accredited RN education program with a diploma, Associate's or Bachelor's degree.
- 2+ years of experience in mental health nursing.
- 1 year of experience in medical surgical nursing preferred.
- Experience in home health care.
