Our organization provides vital services to youth who have arrived in the U.S. without family or guardians. These children are designated as Unaccompanied Children (UC) and often come from challenging backgrounds. They are placed in foster homes or group settings where we offer case management, therapy, and other supportive services.

This program also aids these minors in adapting to American customs and culture while pursuing educational goals, learning English, and achieving independence. As a Foster Care Case Manager for Unaccompanied Children, your responsibilities will include delivering casework and support services to children in foster care, facilitating connections with potential family sponsors, and assisting with sponsorship applications as necessary. You will also be tasked with maintaining effective communication with stakeholders to ensure continuity of care.

This position requires the ability to work independently with moderate supervision while adhering to established guidelines and regulatory requirements.

Key Responsibilities

  • Ensure all youth under your supervision receive necessary services, including mental health, medical, recreational, educational, and other supportive services, in accordance with program documentation expectations.
  • Develop and complete assessments, service plans, progress notes, and incident reports in a timely manner, fostering a trusting relationship with clients to encourage their personal growth.
  • Maintain accurate records on foster children in care, ensuring compliance with internal documentation practices and reporting requirements.
  • Participate actively in weekly staff meetings with clinical teams and stakeholders.
  • Facilitate ongoing education and support for foster parents.
  • Conduct monthly meetings with foster parents and children to address any service delivery concerns.
  • Provide translation services for program clients and staff if bilingual.
  • Communicate effectively with community agencies involved in service provision.
  • Engage in training opportunities and apply new knowledge to service delivery.
  • Prepare for and attend required court hearings, if applicable.
  • Transport clients as needed.
  • Identify community resources and refer families for additional support.
  • Adhere to federal, state, program, and agency regulations.
  • Demonstrate flexibility in working hours and the ability to work independently in the field.
