Position Overview

This role is a full-time remote position, with the expectation of occasional travel to engage with members in the designated area and attend necessary meetings.

Key Responsibilities
  • Conduct thorough assessments of members' health needs through telephonic and in-person interactions.
  • Develop, implement, and coordinate comprehensive case management plans to support members' overall health and wellness.
  • Utilize advanced clinical judgment to facilitate access to appropriate health care services and resources.
  • Engage in crisis intervention for members facing behavioral health or medical emergencies, ensuring they receive timely and effective care.
  • Collaborate with multidisciplinary teams to address barriers to care and enhance quality of services provided.

  • Provide education and support to empower members in making informed health decisions.

Qualifications

To be considered for this position, candidates must possess:
  • A minimum of 3 years of experience in an acute care setting as a Registered Nurse.
  • A current and unrestricted RN license in the state of Michigan.
  • Reliable transportation for travel purposes.

Compensation and Benefits

The compensation for this role typically ranges from $60,000 to $129,615.20 annually, based on experience and qualifications.
