**Responsibilities:**

  • Conduct comprehensive assessments and develop treatment plans to address the unique needs of each consumer.
  • Provide case management services, including crisis intervention, referral, and linkage to community resources.
  • Facilitate group activities and provide outreach services to consumers with multiple needs.
  • Maintain accurate and detailed case documentation, ensuring compliance with CMHA-CEI's policies and procedures.
  • Collaborate with multidisciplinary teams to ensure seamless service delivery and optimal outcomes.

**Requirements:**

  • Bachelor's degree in social work, psychology, or a related field.
  • Minimum one year of professional experience working with adults with serious and persistent mental illness and/or co-occurring disorders.
  • Ability to operate a computer and gain proficiency in CMHA-CEI's computer systems.
  • Effective communication and interpersonal skills, with the ability to work with diverse populations.
  • Valid Michigan driver's license and access to reliable transportation for job-related use.

**Conditional Employment Requirements:**

  • Background verification and credentialing procedure, including a Michigan State Police clearance check, verification of a professional license, Medicaid/Medicare verification, and an acceptable Michigan driving record.
  • Pass a pre-employment drug screen.
