Job Title: Care Coordinator

Estimated Salary: $62,000 - $72,000 per year

About the Role

We are seeking a skilled Care Coordinator to join our team at Sheffield Manor Nursing and Rehab. As a Care Coordinator, you will play a vital role in providing medically-related social services to our guests.

Responsibilities:

  • Complete admissions, social services evaluations, and provide referrals as needed;
  • Assist with financial and legal matters, including applying for pensions and referrals to lawyers;
  • Provide discharge planning services, including arranging intake for home care services and transfer arrangements to other facilities;
  • Ensure accurate and timely documentation in MDS care plans.
Requirements

To be successful in this role, you will need:

  • A current social work licensure in the state;
  • A Bachelor's degree in social work, with a Master's degree preferred;
  • One year of supervised social work experience in a healthcare setting.
