About the Role

We are seeking a Clinical Care Coordinator to join our team at the Detroit Medical Center. As a Clinical Care Coordinator, you will play a critical role in ensuring the seamless coordination of patient care activities.

Responsibilities
  1. Under the supervision of a registered nurse, you will coordinate patient care activities, including patient/family education, for an assigned group of patients.
  2. You will assume the role of care coordinator for uncomplicated patients and utilize available clinical pathways and protocols to ensure efficient care delivery.
  3. Collaborate with physicians, care management specialists, and other healthcare providers to carry out the nursing process.

Requirements:

  • Graduation from an accredited school of nursing.
  • Temporary authorization to practice as a graduate nurse under the direct supervision of a registered nurse.
  • Licensure to practice as a registered nurse by the state of Michigan within 90 days of hire.
