POSITION OVERVIEW

The Licensed Vocational Nurse (LVN) will report directly to the Wellness Director and play a crucial role in managing the Wellness Department, ensuring the health and safety of residents. This includes overseeing medication distribution, maintaining accurate records, scheduling personnel, and developing daily operational tasks. The LVN will also be responsible for organizing resident documentation and maintaining emergency preparedness.

KEY RESPONSIBILITIES

In addition to the responsibilities outlined in the position overview, the following duties are vital for effective performance in this role:
  • Supervise the medication administration process, conducting regular audits of medication logs, ordering medications from the pharmacy, and ensuring proper storage and care of medications.
  • Document significant changes in resident health and communicate these to relevant parties, including any adverse reactions to medications, treatments, or notable incidents.
  • Assist with medication management as required, including medication passes and accurate documentation.
  • Update medical records and care plans, ensuring daily resident documentation aligns with company policies and procedures, including medication distribution, activities, and observations.
  • Participate in required training sessions and report any concerns promptly.
  • Incorporate state and community regulations during training and orientation sessions.
  • Review incident reports and assess the need for interventions to reduce the risk of future occurrences.
  • Schedule Personal Care Assistants and assign daily tasks accordingly.
  • Maintain adequate inventory of medical supplies, ensuring they are always in stock.
  • Conduct weekly follow-ups on bathing schedules and vital signs, including blood pressure, weight, and respiration.
  • Act as the primary contact during shifts for communication with supervisors, healthcare providers, and family members.
  • Foster positive community relationships by demonstrating professionalism and cooperation with residents, families, team members, healthcare providers, and vendors.
  • Contribute to team dynamics by being adaptable in work assignments, providing support, and understanding the interconnectedness of this role with other community positions.
  • Adhere to scheduled activities while maintaining regular attendance and punctuality.
  • Maintain a steady work pace in accordance with the workload.
  • Ensure all health equipment is safe and functioning properly.
  • Assist in lifting and transferring residents as necessary.
  • Address environmental hazards safely and appropriately.
  • Review resident files weekly to ensure all necessary signatures are obtained.
SUPERVISORY DUTIES
  • This position does not include direct supervisory responsibilities.
EDUCATION AND EXPERIENCE REQUIREMENTS
  • Valid and unencumbered California LVN license
  • A high school diploma or equivalent and prior experience in an assisted living or skilled nursing environment are required.
  • Successful completion of Live-Scan Clearance, TB testing, physical examination, and CPR/AED certification is mandatory upon job offer.
  • Certification as a nursing assistant is preferred.
  • Fluency in reading, writing, and understanding the English language is essential.
  • Willingness to work additional hours, including evenings and weekends, is necessary.
