Resident Service Coordinator

As a Resident Service Coordinator at NHE Inc, you will play a vital role in providing exceptional support to residents in our multifamily housing communities. Your primary responsibility will be to proactively educate and provide referrals and services to residents who are frail or at-risk, empowering them to live independently and exercise their right to self-determination.
Key Responsibilities
- Conduct comprehensive, non-clinical assessments of residents to identify their wellness and social needs.
- Develop and sustain partnerships with local service providers, including the Area Agency on Aging and community-based supportive service providers.
- Maintain an up-to-date resource directory with all local service providers and exercise professionalism within the property management team through excellent communication and customer service skills.
Requirements
- Bachelor's degree in social service-related field or meet HUD required work experience.
- Knowledge of and training in elderly and disability services, referral processes, and local services for the elderly and people with disabilities.
- Two or three years of social service delivery experience, preferably working with elderly populations or persons with disabilities.
