POSITION SUMMARY

The primary objective of this role is to oversee and facilitate the assessment and evaluation of residents in compliance with applicable federal, state, and local regulations, as well as directives from management.

KEY RESPONSIBILITIES

  • Manage the RAI process, including the completion of MDS, CAAs, and the formulation of individualized care plans for residents, adhering to RAI protocols and facility guidelines.
  • Ensure the interdisciplinary team adheres to the resident care plan and monitor their progress to maintain compliance with the MDS process.
  • Accurately complete medical documentation, charts, and reports in a timely manner.
  • Assist the Director of Nursing (DON), Assistant Director of Nursing (ADON), or supervisors as needed.
  • Understand the long-term care reimbursement mechanisms, including PDPM, Managed Care, and Medicaid.
  • Participate in the scheduling of resident care meetings.
  • Engage in the Quality Assurance and Performance Improvement (QAPI) initiatives.
  • Monitor Quality Care Indicators and implement quality enhancement strategies.
  • Adhere to all facility policies and procedures, including safeguarding user ID codes and passwords.
  • Report any occupational exposure to blood, body fluids, or hazardous materials to a supervisor immediately.
  • Participate in facility inspections and assist with corrective action plans as necessary.
  • Attend meetings and contribute to committees as requested.

QUALIFICATIONS

Education / Licensure

  • Registered Nurse (RN) or Licensed Vocational Nurse (LVN) degree from an accredited institution.
  • Current RN or LVN license in good standing.
  • Valid CPR and BLS certification.

Experience / Skills

  • At least one year of experience in skilled nursing is preferred.
  • Proficient in speaking, reading, writing, and understanding the English language.
  • Ability to utilize a computer or tablet for resident data entry.
  • Familiarity with PCC is advantageous.

WORKING CONDITIONS

  • Frequent interruptions may occur.
  • Interaction with residents, families, and regulatory agencies is expected.
  • May require working beyond scheduled hours occasionally.
  • Potential exposure to infectious waste and diseases, including TB and bloodborne pathogens.

PHYSICAL REQUIREMENTS

  • Ability to move throughout the facility during the workday.
  • Engagement in nursing area activities.
  • Repetitive hand movements may be necessary.
  • Capability to read fine print on tablets, progress notes, and medical labels.
