REGISTERED NURSE - LEVEL OF CARE (LOC) RN

Location: Reseda, CA
Compensation: Up to $108,000 annually
Work-Life Balance: Enjoy a schedule of Monday to Friday, 8:30 AM to 5 PM with no on-call responsibilities.
Qualifications Required:
Bilingual proficiency in Spanish and English.Graduation from an accredited nursing program (ASN, BSN, MSN).Current RN licensure in California.Certification in CPR (BLS) and first aid.Possession of a valid California driver's license and access to a reliable vehicle.A minimum of one year of RN experience in acute care settings, preferably with the elderly.Confidence in delivering in-home care to frail and elderly patients.Strong computer skills (MS Office) and excellent communication abilities.As a Level of Care RN, you will conduct assessments in private residences, ensuring that all necessary documentation is completed for state approval. You will provide both direct and indirect nursing care to participants enrolled in the PACE (Program of All-Inclusive Care for the Elderly) program, collaborating closely with the RN Clinic Supervisor and Primary Care Physician to manage complex clinical scenarios.
Key Responsibilities:
Participate in interdisciplinary team meetings to discuss prospective enrollee cases.Conduct initial home evaluations for Level of Care assessments.Complete and submit initial and annual Level of Care assessments to the state.Document participant assessments and care provided in medical records.Prepare for bi-annual LOC audits by reviewing necessary documentation.Monitor participants' health and social needs, addressing immediate concerns and coordinating referrals.Assist in developing comprehensive Plans of Care that reflect participant and family needs.Deliver direct nursing care as assigned, including wound care, IV therapy, and vital sign monitoring.Educate participants and families on health management and self-care strategies.Communicate health findings and changes to the Primary Care Physician.Ensure all ordered tests and diagnostics are completed as directed.Manage medication coordination and compliance for participants.Why Choose Us:
