Job Title: Registered Nurse - Urogyn Care Network

Job Summary:

We are seeking a skilled Registered Nurse to join our Urogyn Care Network team. As a Registered Nurse, you will be responsible for providing high-quality patient care, working closely with physicians and staff to coordinate care, and serving as a liaison between patients, staff, and physicians.

Key Responsibilities:

  • Obtain patient consent for care and ensure patient confidentiality
  • Obtain and document health history, information on chief complaint, vital signs, and health risk factors and assess disease condition
  • Provide patient care based on practice guidelines, standards of care, and federal/state laws and regulations
  • Execute prescribed treatments and medical interventions, administer prescribed medications, and monitor and document treatment progress and patient response
  • Delegate as allowed under the Nurse Practice Act. Refers to/consults with physicians, other health providers, and community resources to prevent/resolve problems or concerns
  • Educate patients and families about health status, health maintenance, and management of acute and chronic conditions
  • Document patient assessment and intervention data using established medical record forms/automated systems and documentation practices
  • Participate in multidisciplinary teams to improve patient care processes and outcomes
  • Tracks quality assurance data and monitors for acute and chronic care management
  • Maintains/reviews patient records, charts and other pertinent information
  • Posts tests and examination results
  • Responds to/refers incoming patient treatment-related phone calls
  • Maintains patient confidentiality
  • Performs other duties assigned

Requirements:

  • Must be an actively licensed Registered Nurse in the state of Illinois
  • Minimum of one year professional nursing experience
  • Completion of nursing training from an accredited school of nursing
