Patient Navigator

We are seeking a highly skilled Patient Navigator to join our team at the University of Miami. As a Patient Navigator, you will play a critical role in facilitating patients' ability to obtain services by identifying barriers to care, coordinating the development of treatment plans, and monitoring the implementation and follow-up of services ordered by caregivers.
Key Responsibilities:- Identify and assess assigned patients to facilitate MyChart account activation and evaluate MWC assessment completion on a daily basis.
- Provide individualized assistance to patients, families, and caregivers to help overcome health care system barriers and facilitate informed decisions in a culturally competent manner.
- Communicate with designated family members or caregivers to assist patients in navigating the patient portal and locating MWC assessments.
- Provide daily reports with documentation of patient communication, conversation, and instructions provided.
- Work closely with the My Wellness Check and Cancer Survivorship Program Teams to support training and administrative efforts.
- Three (3) years of relevant experience in a medical office or hospital setting.
- Excellent patient experience and communication skills.
- Ability to build patient rapport and provide compassionate care.
- Basic oncology-specific knowledge and cultural sensitivity.
- Compliance with all regulatory patient privacy guidelines (i.e. HIPAA).
