Patient Navigator

We are seeking a highly skilled Patient Navigator to join our team at the University of Miami. As a Patient Navigator, you will play a critical role in facilitating patients' access to healthcare services, identifying barriers to care, and coordinating treatment plans.
Key Responsibilities:- Provide individualized assistance to patients, families, and caregivers to overcome health care system barriers and facilitate informed decisions in a culturally competent manner.
- Identify and assess assigned patients to facilitate MyChart account activation and evaluate MWC assessment completion on a daily basis.
- Communicate with patients, families, and caregivers to provide instructions for activation of MyChart account and navigation of patient portal to locate MWC assessments.
- Collaborate with healthcare professionals to ensure population-specific care to patients involved in the My Wellness Check Initiative (MWC).
- Provide daily reports with documentation of patient communication, conversation, and instructions provided.
- Support the continuous evaluation and optimization of MWC via development, tracking, and reporting of MWC patient satisfaction surveys.
- High School diploma or equivalent.
- Three (3) years relevant experience in a medical office or hospital preferred.
- Excellent patient experience/communication skills.
- Ability to effortlessly build patient rapport.
- Basic oncology-specific knowledge.
- Cultural sensitivity.
- Compliance with all regulatory patient privacy guidelines (i.e. HIPAA).
- Compassionate.
- Goal-driven.
- Ability to recognize, analyze, and solve a variety of problems.
- Ability to maintain and grow effective interpersonal and professional relationships.
- Ability to communicate effectively in both oral and written form.
- Excellent attention to detail and organizational skills.
