Patient Navigator

Join the University of Miami Health System as a Patient Navigator and play a vital role in ensuring patients receive the best possible care. As a Patient Navigator, you will be responsible for facilitating patients' access to services, identifying barriers to care, and coordinating treatment plans.
Key Responsibilities:- Conduct patient evaluations to gather information necessary for effective treatment planning
- Ensure patients have access to insurance and social service programs for which they are eligible
- Provide patient reminders of upcoming appointments and serve as a liaison between patients and providers
- Identify and resolve real or perceived barriers to care
- Educate and counsel patients on health-related issues and life situations
- Maintain current knowledge of community health resources and programs available to patients
- High School diploma or equivalent required
- Minimum 3 years of relevant work experience required
- Experience in a medical office or hospital preferred
