Patient Navigator

The University of Miami is seeking a highly skilled Patient Navigator to join our team. As a Patient Navigator, you will play a critical role in facilitating patients' access to healthcare services by identifying barriers to care, coordinating treatment plans, and monitoring the implementation and follow-up of services ordered by caregivers.
Key Responsibilities:
- Conduct patient evaluations to gather information necessary for effective treatment planning.
- Ensure patients have access to insurance and social service programs for which they are eligible.
- Provide patient reminders of upcoming appointments and serve as a liaison between patients and providers.
- Identify and resolve real or perceived barriers to care.
- Educate and counsel patients on health and life situations.
- Maintain current knowledge of community health resources and programs available to patients.
- Adhere to University and unit-level policies and procedures, safeguarding University assets.
Requirements:
- High School diploma or equivalent required.
- Minimum 3 years of relevant work experience required.
- Ability to process and handle confidential information with discretion.
- Effective communication skills in both oral and written form.
- Ability to handle difficult situations with professional composure.
- Maintain effective interpersonal relationships.
- Ability to understand and follow instructions.
